Abstract

Discharge planning penting untuk menjamin kesinambungan perawatan pasien dari rumah sakit ke rumah. Pengkajian awal di Ruang Rawat Inap Eboni menunjukkan edukasi pasien pulang masih dominan verbal, belum terstruktur, belum menggunakan teach-back, dan belum terdokumentasi sistematis. Tujuan studi kasus ini adalah mendeskripsikan pengembangan dan simulasi awal DP-TB Checklist sebagai inovasi manajemen keperawatan. Studi kasus dilakukan di Ruang Rawat Inap Eboni Rumah Sakit Universitas Andalas melalui observasi 14 proses edukasi pasien pulang, telaah 14 dokumen pasien, wawancara singkat dengan kepala ruang, perawat, pasien, dan keluarga, serta simulasi awal pada 6 pasien. Analisis menggunakan pendekatan 6M, fishbone, SWOT/TOWS, dan POAC. Edukasi obat diberikan pada 100% pasien dan jadwal kontrol 85,7%, tetapi edukasi tanda bahaya hanya 35,7%, perawatan mandiri 42,9%, keterlibatan keluarga 42,9%, teach-back 0%, dan penggunaan checklist 0%. Simulasi awal menunjukkan checklist terisi lengkap 83,3%, teach-back 83,3%, keluarga hadir 83,3%, dan QR code berhasil diakses 66,7%. DP-TB Checklist layak sebagai instrumen sederhana untuk menstandarkan edukasi pulang, memverifikasi pemahaman pasien-keluarga, dan memperkuat dokumentasi mutu. Implementasi berkelanjutan memerlukan SOP, champion, briefing perawat, dan audit berkala.
